I was thinking the same, that when customer ignorance becomes common enough 
you start assuming that the next person who orders doesn't know what they want 
and make what you think they want.  I've only made the classic drink for one 
customer (she liked it on the rocks in a collins glass) although she likes 
them so much that she comes back and asks for me so I've made them for her a 
few times.  

Even when I'm thinking that they're going to end up with a rum spiked 
strawberry smoothie I try to let on that that's different than what they 
ordered.  I might ask "Did you want that up or on the rocks?" to which I 
normally get "Isn't it blended?"

"Of course it is, I'll be right back"  And off I go, looking for strawberry 
puree.
