Not sure on the regional thing, but I have found those that drink rum and coke 
with a wedge of lime don't always know it is a Cuba Libre ... ditto for a Gin 
Rickey.  I had a regular that liked a specific gingerale and preferred vodka 
that never called it a Moscow Mule because of confused bartenders.  It was 
just easier to say "I'll have a Skyy and ginger with a lime please."
 
I'm not sure if any of this helps, or makes sense, but I hope it does.
