I guess I should have explained the context there for non-Americans. The
culture here definitely makes it very easy to forget that other people exist.
Johnny Carson was a popular TV host for several decades, and he did a comedy
routine in which he dressed up as a mind reader, was handed a sealed envelope,
"magically" answered the question contained within, and then would open the
envelope and read what the question was. So in this case he picks up the
sealed envelope, holds it to his forehead, and says, "Double Trouble." The
audience sits for a minute, wondering what the question is, at which point he
opens the envelope and says, "What is the name of a drink which is half beer
and half prune juice?"
