I've only worked for one place that offered health insurance and it was a 
coporate restauarant/bar environment. However, we still had to pay for it, if 
I remember correctly, it amounted to about $100 a month being deducted from my 
check, and you had to work over 25 hours a week to qualify for it. Though, it 
wasn't too bad of insurance, and included dental (which covered 80% of most 
work, and 50% of cosmetic work, to a max of $2000 a year coverage, with a $250 
deductable). Anyways, nowadays, offering even partially subsidized health care 
for employees is a very expensive option.
