Well, call and premium (top-shelf) refer to the type and quality of the 
alcohol used to make the drinks.  This "quality" is at every bar's discretion. 
 So, say you order a Gin Tonic.  If you don't specify and are not asked, the 
bartender will make it with whatever their particular "well" variety gin is... 
say Beefeater (probably something even cheaper but you'd have to ask a 
bartender).  Say you order a "Jack and Coke."  That's a call drink, as you 
specified what type of alcohol you want in the drink... as opposed to ordering 
a "whiskey and coke," which would probably yield you coke with some kind of 
Canadian Whiskey.  Now, if you want the absolute best (and priciest) alcohol 
used in your drink then order a "top-shelf" drink.  So, a top-shelf margaritta 
will be made with the best tequila in the bar.
