Here is a copy of the contract I'm using. It is for the state of Minnesota, so 
check with your state laws and city ordinances and adjust this one accordingly.
From the attorneys, insurance agents, city officials and information I found 
on the state sites, basically, you want to cover your ass from being sued.
The main condition on this one (for the state of Minnesota) is the 
host/hostess having insurance. The attorneys I spoke with (my wife is a 
para-legal, I know alot of lawyers), as long as they say they have insurance, 
it will release you from responsibility for accidents/injuries the guests may 
obtain during or after the party. After all, the host/hostess is signing this 
contract stating they have insurance, and most home-owners policies have 
coverage for alcoholic parties.
Again, this is for the state of Minnesota.

As the host or hostess, I agree to the following conditions: 
·	Minors will not be served any beverages containing alcohol.
·	All guests are subject to being "carded" for age verification before being 
served.
·	The bartender reserves the right to refuse alcohol to anyone he feels may 
have had too much to drink.
·	I assume all responsibility for all guests and their actions.
·	With respect to this social function/event, I declare that there is in 
effect an enforceable policy of homeowners insurance/establishment insurance.
·	Payment by check is due in full (less deposit) ten business days prior to 
function/event. Payment by cash is due prior to bar set-up.
·	A $50.00 non-refundable deposit is required for each bartender at signing of 
this contract.
·	Fee for services for a three hour period is $________ (less deposit) per 
bartender.
·	The fee for each additional hour is $________ per bartender, to be paid at 
the end of the function/event (cash only) to the bartender(s).
·	A tip jar may be placed on the bar/table.
·	The bartender is not responsible for, and will not purchase or supply any 
alcohol for this function/event.
·	Reimbursement to the bartender shall be paid for any non-alcoholic items 
purchased for this function/event. The bartender will provide receipts for 
items purchased and be reimbursed for that amount at the end of the 
function/event (cash only).
Host/Hostess Responsibilities:
·	Purchase all alcohol, mixes and extras.
·	Provide a bar or table for the bartender near a sink or kitchen.
·	Provide security for the function/event (if city ordinance or state law 
requires).
I have read and agree to the conditions listed above.
