We too enjoy that confusion around here. Many times you hear patrons ordering 
a drink "straight up." 99% of the time they just want it served "up" - 
(chilled in a shaker and either stirred or shaken and then strained into the 
appropriate glass.)

However, when our patrons order "tequila straight" they are usually referring 
to "neat."

Mostly depends on the type of liquor ordered as to what they really mean.
