I worked in one place that had what they called an asshole botton that added 
$4 to the bill every time  billed as Associate Sevvice Surcharge.The few times 
that ithat it was used and somewould ask the manager  he'd comp something to 
the party and talk to the wait staff  about why the button was used and in no 
case did the $4 charge make it into waiters pocket.

Most of the places I've worked have had such a policy footnoted on the menu.

I personaly would always tell the people about it.Some people in the service 
profession have been know not to  tell and double dip when,then tipped  on the 
amount of check/gratuity inc. I've have found that generaly I was better off 
if the establishment didn't have that policy.I learned tha I could increaw my 
tips byt telling the bill payer "the gratuity is included it amounts to so and 
so"
