If a name is connected to only who submitted the drink than you don't have to 
put any name connected to any drink that already exists on the web.

The older drinks probrably have a better history of who the creator was. A 
Gibbson would be an example.  This website on some drinks include a little 
history.

As far as distribution. I would first try the author of the software and see 
if he will distribute yours.

I don't know.  I mailed the list to Cheryl some time ago and haven't heard 
anything.
